If you have questions or would like to share your thoughts, give us a call or send us an email. Your first call will be complimentary, and will have no cost to you.
The purpose of the initial, complimentary phone consultation will be to get to know one another, hear your concerns,
and help determine how we can best meet your needs. You’ll have an opportunity to share your concerns
and to ask questions.
At the end of the phone consultation, we will discuss next steps, including scheduling an initial diagnostic assessment or providing you with referrals in the community which may be more convenient or helpful for you.
Before Your First Appointment
We will send you an “Invitation” to log in to our secure online system. We’ll then send you a packet including our intake questionnaire and consent form. We recommend completing these online before our first appointment. I will try to have the questionnaires scored and will be able to share the results with you. Although the various questionnaires and measures are rather lengthy, they can be quite helpful in developing a treatment program for you. They greatly accelerate the treatment process and allow us to monitor improvement over time. If possible, I would appreciate receiving medical and (if appropriate) school records and reports of prior treatment.
